Public health departments can be involved in many different aspects of a community's life. Some of their functions are popular with residents - for example, SF Sunday Streets with healthy food, activities, health information, and live music. A lot of people participate! Other times the enforcement efforts of public health departments might seem intrusive. For example, in the city where I live, Richmond, it is not legal to smoke inside or near an apartment or condo, to protect the rights of the nonsmokers to breath clean air. Some people like this law, and some people hate it!
As noted above, many important public health organizations are non-governmental organizations -- independent nonprofits or professional associations. Really big ones may operate in many states or nations (like the Red Cross or the American Lung Association), while others work in specific communities or regions (like the San Francisco AIDS Foundation). Universities and other research centers also play an important role in public health.
