When assisting the provider with prescriptions, the MA must verify that the following information is accurate and complete:
- Date the prescription is issued
- Patient’s name and address
- Patient’s date of birth
- Provider name and business address
- Drug name
- Drug strength
- Drug form
- Quantity to be dispensed by the pharmacist
- Directions for use
- Number of refills
- Signature of provider
- Provider’s DEA number (for narcotic prescriptions)

The image below shows an example of a blank prescription from a pre-printed prescription pad. These forms are not used for e-prescribing but are delivered in person by a patient to a pharmacist. Prescription forms are available through specialty printers and must be printed on prescription security paper. Prescription pads are considered a high-theft item and must never be left unattended or used for scratch paper. Prescription pads, when not in use, should be stored in a locked desk or cabinet.
Typically, the provider’s name, address, and contact information are printed at the top of the prescription form, and areas are provided for the patient’s name, address, and date of birth. The middle of the form contains a space for drug information, including the name, strength, form, and quantity of the drug to be dispensed by the pharmacist. The directions for use and number of refills permitted are also written in this space by the provider or MA. Some prescription forms include information to indicate that the pharmacist can dispense a generic drug instead of a brand-name drug. The provider’s signature and National Provider Identification Number are located at the bottom of the form. The MA can assist the provider in preparing the prescription, however, the MA is not permitted to sign the prescription. Prescriptions, whether on paper or electronic, must be documented in the patient's record.
