Otitis externa can be caused by numerous factors, including bacterial or fungal infections, and it is typically associated with activities that involve water exposure, such as swimming. The most common cause of otitis externa is bacterial infection, often due to bacteria such as Pseudomonas or Staphylococcus aureus. These bacteria thrive in a moist environment, making the ear canal susceptible to infection after exposure to water. Fungal otitis externa, caused by species such as Aspergillus or Candida, can also occur, especially in individuals with underlying skin conditions or weakened immune systems. Scratching or inserting foreign objects (like cotton swabs) into the ear can damage the ear canal's delicate skin, creating an entry point for infection. Allergic reactions to certain substances, such as hair products or earplugs, can cause inflammation and contribute to otitis externa.
Otitis externa typically presents with ear pain or discomfort, often worsened by pulling on the ear or pressing the tragus (the small, pointed part in front of the ear canal). Other symptoms include itching, redness, swelling in the ear canal, drainage of clear or pus-like fluid, decreased hearing, and pain when moving the jaw or opening the mouth. An otoscope is used to examine the ear canal and assess any inflammation, discharge, or blockage. Treatment for otitis externa aims to alleviate symptoms, resolve the infection, and prevent complications. Common treatment options include antibiotic or antifungal ear drops, over-the-counter pain relievers such as acetaminophen or ibuprofen, avoidance of water, topical corticosteroids, and oral antibiotics.
