    Medication Adherence Case Study #1

    AW is 50-year-old with heart failure (HFrEF Stage C NYHA Class II) is taking metoprolol, furosemide and
    lisinopril. AW continues to work as a court reporter and cannot leave the proceedings once court is in
    session. Create a medication plan for frequency and timing of medications around AW’s work schedule
    yet optimizing the pharmacotherapy. Please consider that during the incident hospitalization for heart
    failure, AW had an episode of hypotension. Also, AW has carpal tunnel syndrome and has a long
    commute to work (no available restroom). AW has been skipping the furosemide and taking metoprolol
    and lisinopril together on route to work; sometimes arriving to the courthouse feeling lightheaded as
    AW goes through the metal detector.

    Assigned Student Task #1.1:

    1. Suggestion a schedule for AW to take their medications that will allow AW to take all
    medications daily, safely arrive at work, and minimize hypotensive episodes. Write a brief
    description of the pharmacokinetics of the various drugs that you considered when creating the
    medication schedule.

    Medication Adherence Case Study #2

    CZ is 30-year-old with heart failure post COVID infection (HFrEF Stage C NYHA Class II) is taking
    metoprolol, furosemide, and sacubitril/valsartan (Entresto). CZ has returned to work and bikes about 3
    blocks to the office. Previously on the weekends CZ rode about 10 miles on a bike trail and wants to
    gradually try resuming longer bike rides. Create a medication plan for frequency and timing of
    medications around to optimize the pharmacotherapy and limit skipped or missed doses. CZ has been
    missing the furosemide and then taking metoprolol and Entresto together at work.

    Assigned Student Task #1.1:

    1. Suggestion a schedule for CZ to take their medications that will allow AW to take all medications
    daily, safely bike to work, minimize hypotensive episodes, and resume biking. Write a brief
    description of the pharmacokinetics of the various drugs CZ is taking, which will help you to
    create the medication schedule.
