- Go to the Alameda County Exercise Safety Guidelines webpage below and read the Physical Activity Readiness Questionnaire (PAR-Q).
- Summarize the information from the PAR-Q (for example, I answered “yes” to __ questions and “no” to __ questions). If you answer “Yes” to one or more questions, you may need to consult with your healthcare provider prior to beginning the exercise phase. Therefore, you must explain any “Yes” answers and if you have already been cleared by your healthcare provider to exercise.
- Save the your answers using a word processing program and upload your saved document to the course.
